Please complete the form to submit non-emergency HomeLife Maintenance Requests. An Estridge representative will email you within 24 hours to acknowledge receipt of your e-mail. You will be contacted within 3 days to discuss your request, schedule an inspection or confirm that the item will be added to your next HomeLife inspection. If we determine to involve a vendor partner to make a repair prior to your next HomeLife inspection, please allow a 10 day lead time for this scheduling to occur.

Do not submit emergency requests on this homeowner form. All emergency requests must be communicated by phone to ensure the most accurate and efficient exchange of information. Contact the HomeLife Department 24 hours a day at 317-582-2444 to report an emergency. An Estridge HomeLife Representative will respond within 2 hours.

Examples Requests Include:
  • Total loss of heat
  • Total loss of air conditioning when the outside temperature exceeds 90 degrees F.
  • Plumbing leak that requires the water to the entire home to be shut off.
  • Total loss of hot water.
  • Total blockage of the sewage system.
  • Total loss of electricity (not a widespread outage).
  • Roof leak that is causing damage to surrounding materials.
  • A security issue; an accessible door or window will not lock.
  • Refer to your HomeLife Maintenance and Reliability Program Manual for a complete list of emergency items.
